At Opsam Health, we have a Sliding Fee Program available for all patients, uninsured or insured. Eligibility is based on household income and family size. If you qualify, you may pay a reduced cost for most services. 

 

You may be eligible for this program even if you have insurance. Co-pays and deductibles are collected at time of visit for all insured patients. If you are unable to pay your co-pay or deductible, we encourage you to inquire about the slide fee or other financial assistance programs we offer. 

 

Applications are available at the reception desk at each location or you may click the link below to print the form and submit to any of our clinic locations . All information on the application is kept confidential. We have designated staff available to help you with completing these applications.

 

To apply for the Sliding Fee Program, please complete the Opsam Slide Fee Application Form AND any proof of income, such as recent pay stub or most recent income tax return.
